Nel Giappone pre-moderno i massaggiatori erano tutti ciechi affinché non potessero vedere i corpi dei loro clienti. Ma nessuno poteva usare una katana così bene come Zatoichi, un samurai nell'animo che non disdegna di barare al gioco dei dadi per arrotondare lo stipendio. Durante un viaggio il ricco Sukegoro resta affascinato dal suo incontro con Zatoichi e lo invita ad andarlo a trovare quando lo desidera. Giunto nel paese natale di Sukegoro, Zatoichi si ritrova invischiato in una guerra tra clan rivali.
